I've had something similar happen to me in two places on the map. One was in the creek near the "Old Farm" down by the docks. The other was in the creek between Gabe's Ranch and the Forest behind it. In both instances trying to cross the creek led to situation where I was unable to exit the water and had to use the teleport home button.
